Follicular Unit Transplantation is the original method for hair transplant surgery. Strip-based, leaving a linear scar at the donor site, not often the first recommendation for male patients these days. But for certain patients, it remains the most clinically appropriate choice, and dismissing it entirely because of its age would be a mistake.

FUT involves excising a strip of scalp from the donor area, typically across the back of the head. That strip is then dissected under microscopic magnification by the surgical team, who identify and prepare individual follicular units from the tissue. Those units are then implanted into the recipient area in the same way as FUE.

The donor site is closed with sutures, leaving a single linear scar. In most patients this heals to a thin, pale line that is not visible when the surrounding hair is kept at a natural length.

Why FUT Is Still Relevant

There are two situations where FUT is genuinely preferable to FUE, and both come up regularly in practice.

The first is patients who do not want to shave the donor area. Women considering a hair transplant, and transgender patients undergoing hair restoration, often fall into this category. FUT allows the strip to be excised without shaving, with surrounding hair simply parted and replaced over the site afterwards. With FUE, the donor area must be trimmed short enough for the punch to work accurately.

The second is graft yield in a constrained donor area. When a patient’s donor supply is limited and they need the maximum viable follicles from a single session, strip dissection under magnification allows the team to identify every usable unit within the tissue. FUE, which relies on selecting and extracting follicles individually from the scalp surface, may leave some viable grafts behind that strip dissection would have captured.

FUT vs FUE: What the Difference Actually Means for You

The most common reason patients choose FUE over FUT is scarring. FUE leaves multiple small round healing points rather than a single linear scar. For men who wear their hair very short at the back, this is a meaningful difference.

But the trade-offs are real. FUT:

• Does not require donor area shaving
• Can yield a higher total graft count in a single session
• Allows the surgeon to cherry-pick the healthiest follicular units under magnification
• Has a longer healing time at the donor site while the suture line settles

FUE:

• Leaves no linear scar
• Shorter initial donor area recovery
• Requires the donor area to be shaved short
• Individual follicle selection is done at the extraction stage, which is efficient but less controlled than strip dissection

For most male patients with no strong preference for very short hair at the back, FUE is the more convenient option. For those where donor sparing or donor-area appearance is the priority, FUT may be the better clinical choice.

What FUT Is Used For at Our Newcastle Clinic

FUT is not limited to patients dealing with full scalp hair loss. It is also used for hairline restoration in patients who want to avoid shaving, and for crown transplants where maximum graft yield from a single session is the priority. Patients who need both hairline and crown work in one visit often find the strip yield from FUT allows this more comfortably than FUE.

For beard transplants and eyebrow transplants, FUE remains the standard as the donor harvest is smaller, though FUT may be discussed if the patient has limited scalp donor availability alongside facial hair goals.

Recovery After FUT

The suture line at the donor site is the key consideration. It takes roughly ten to fourteen days to heal to the point where normal activity can resume without risk to the wound. Sutures are typically removed at around ten days.

The recipient area recovery follows the same pattern as FUE. Grafts remain vulnerable for the first two weeks, gentle washing begins on day three, and transplanted hairs shed before new growth begins from around month three. Final results are visible at twelve months.
