A transgender hair transplant is a procedure focused primarily on feminisation of the hairline. For trans women and non-binary patients who have experienced male pattern hair loss, or who have a naturally high or masculine hairline, transplantation can be one of the most meaningful steps in the gender-affirming process.

At Hair Transplant Newcastle, we approach these procedures with the same clinical rigour as any other transplant, and without assumptions about what the patient wants. Every consultation starts with an open conversation about goals, timeline, and what is genuinely achievable.

Dr Aziz Elgindi (GMC: 7450896) and Dr Mohammed Ditta (GMC: 7084372) have experience working with patients across the spectrum of gender identity and expression. For trans men, a beard transplant is often the primary surgical goal, and that is equally supported at our Newcastle clinic.

 

What the Procedure Addresses

A feminising hairline transplant typically involves one or more of the following:

  • Hairline lowering: Creating a lower frontal hairline that sits higher up the forehead than a typical male hairline
  • Temple softening: Rounding off the angular temple recession that male pattern hairlines develop, creating the softer, rounder temple corners associated with a feminine hairline
  • Density restoration: Rebuilding areas where male pattern hair loss has caused thinning or recession along the hairline

These goals can be addressed individually or in combination. The most common request is a combination of temple softening and frontal hairline lowering, which together significantly change the overall frame of the face.

How Feminising Hairline Surgery Differs

The most important difference between a feminising hairline and a standard male hairline restoration is the shape.

Male hairlines typically sit higher on the forehead and have a more defined M-shape, with angular temples. Feminine hairlines sit lower, have rounded temple corners, and often have a slight oval curve rather than a flat line across the front.

Creating a feminine hairline on a patient who has experienced male pattern recession requires:

  • Designing a hairline shape that is appropriate for the patient’s facial structure
  • Filling in the temporal corners to soften the angular recession
  • Using single-hair grafts at the very front of the hairline to create a soft, natural edge rather than a harsh line
  • Accounting for ongoing hairline maintenance requirements, particularly for patients earlier in their hormone therapy journey

Hormone Therapy and Hair Transplant Timing

For trans women on oestrogen-based hormone therapy, HRT often reduces ongoing hair loss significantly. Many patients find that further frontal recession stabilises or even partially reverses on HRT.

Where possible, it is worth allowing HRT sufficient time to demonstrate its effect on hair loss before planning a transplant. This prevents a situation where transplantation is done before the extent of hormone-related recovery is known. However, this is not a strict requirement. Some patients have experienced significant loss before starting HRT, or have loss patterns that are not responsive to hormones, and proceeding with a transplant earlier makes clinical sense.

Graft Count and Technique

Graft counts for feminising hairline procedures typically range from 1,500 to 3,500 depending on the extent of the work required. Temple softening alone may require 600 to 1,200 grafts per temple. Full hairline lowering with temple rounding sits at the higher end.

FUE is the standard technique, with DHI preferred for precision work along the hairline and temple zones where new follicles need to sit between existing hairs. For patients who cannot or do not wish to shave the donor area, FUT provides an alternative that avoids donor area shaving.

No. You are welcome to attend for a consultation and discuss your goals in whatever terms feel right to you. Your surgeons will assess the clinical picture: your current hairline, donor supply, and what you want to achieve, and advise accordingly. There are no requirements or assumptions about disclosure.

Generally yes, though it depends on the specific medications and your medical history. Your surgeon will review your current medications at consultation. Some anticoagulants and other medications may require a pause before surgery. HRT itself is not typically a contraindication to hair transplant surgery.

FUE requires the donor area at the back and sides of the scalp to be trimmed short. For patients who prefer not to shave any part of their head, FUT (the strip method) is an alternative. The strip is taken from beneath the surrounding hair, which falls back over the donor site and covers it during healing. This is a widely used approach for trans and non-binary patients who prefer to avoid visible shaving.

The transplanted hairs shed between weeks two and five after the procedure, which can be unsettling but is completely normal. New growth begins around month three. By month six, the improved hairline shape is clear. The full result, including final density and texture, is visible at ten to twelve months.

Combined hairline and crown procedures are possible. Whether it makes sense for your specific case depends on your donor supply and the extent of the work required at the hairline. Your surgeon will advise whether a combined approach or staged procedures is more appropriate.

It was an outstanding experience from start to finish. Naturally, I was unsure on which organisation to choose for the procedure, but HSHTC came across as professional and caring, as well as having a wide range of availability. There are negative reviews on all businesses, and this drives customers like myself away. However, something told me this would likely be someone with unrealistic expectations, or not reading into the procedure as a whole. I chose to ignore the negatives and read the positives. Sara the coordinator was absolutely amazing! She outlined everything and made it clear of what is expected and what is not. After a great consultation, I was passed to the next stage of meeting surgeon Dr Ditta. He presented extremely knowledgeable and passionate about what he does, so I decided to go ahead with the procedure. The surgery day was expected to be a long, but rewarding day. It started with having to shave my head (mandatory requirement for this procedure). I’d say to anyone the thought is worser than the event, and to just do it for the positives that will follow. Dr Ditta and his team were extremely caring and sensitive, especially with briefing before any painful situations pre anaesthetic. After that, I was allowed to top up when requested and leave them to do what they do best. The day as a whole was enjoyable and interesting to speak to the team to learn more about the procedure and get to know them. Lunch was provided (yay). It was lovely of them to offer, and you definitely need to eat mid procedure to keep energy levels. At the end of the day, I was given clear instruction of how to have the best success rate. It was through a video and in person. If you have questions, ask as it’s what the team is there for. The first few days of recovery can be quite awkward and uncomfortable, but I’d recommend booking 2 weeks off work to fully recover and let the grafts set. Once you get use to the sleeping situation, it’s easy and a lovely break from reality (put pillows below your knees and stack below head for pure comfort!). The aftercare products was extremely useful and easy to use. The grafts will fall out after a few weeks, it’s expected and you’re told this plenty of times!! Having a healthy lifestyle makes your hair grow quicker and thicker, so swap your diet to good healthy hair foods. When your transplanted hair grows back, it’s an amazing feeling, but only the beginning. As the months pass by, you’ll notice a massive improvement. During this process, you’ll have regular aftercare and the chance to raise any concerns. I personally didn’t have any issues and was just grateful to still be in contact and told everything is going as it should. I’m 6 months post surgery and I’m seeing the benefits hugely, and it’s not even reached maximum results yet, as this is one year. Thanks for an amazing service!

Having given some time and thought about having a Hair Transplant, I contacted Harley street hair transplant clinics. The consultant (Sara) explained everything regarding the procedure with Dr.Ditta and the technicians. From the beginning, Dr.Ditta explained the days events and how the procedures would be done beginning with taking a photo of my head and showing me where the grafts would be taken from and where they would be placed. The procedures would be done and completed in two parts, with a lunch break in between. Once completed, Dr.Ditta went through the aftercare, explaining what to do each day for 10 days. I was given capsules, shampoo, sponge, neck pillow for sleeping with and information regarding how to look after my hair. Dr. Ditta and his team were welcoming and friendly ensuring I was looked after throughout both parts of the procedure. Sara maintained regular contact to make sure recovery was going well. The results were great, patience is needed as the grafts start to grow, and then you can see the results. Extremely happy with everything, from start to finish.

Would highly recommend! Everything went very smoothly, communication was excellent throughout. I travelled from Edinburgh to Manchester for the procedure and all travel and accommodation costs were covered. Dr Ditta explained everything clearly and delivered clear and detailed post-op instructions. I was delighted with the whole experience and results so far are very promising!

Overall, my experience at the clinic was positive. Dr. Ditta was exceptional — he made me feel comfortable and at ease throughout the procedure, and his professionalism was truly reassuring. However, I was disappointed with one member of the team who, in my opinion, did not fully follow health and safety protocols. She had her hair down without a surgical cap and was using her phone while wearing gloves, then proceeded to touch my hair without changing them. This raised concerns about hygiene and cross-contamination. Her manner was also not very pleasant, which made the experience less comfortable. Although I was initially worried about the risk of infection, I felt more at ease by the end of my visit. I hope the management reviews this matter to ensure that strict hygiene and professional standards are consistently maintained. Aside from this, I was very satisfied with Dr. Ditta’s care and look forward to seeing my results.

Before the procedure I had online and face to face consultations with no obligations. These gave me all the information I needed and really put me at ease and gave me the confidence to proceed. Everyone was really knowledgeable and friendly and clearly very experienced. Booking the procedure was quick and easy and I got a date that suited me. The procedure itself went really smoothly I was able to put headphones in and listen to podcasts and was very comfortable. Afterwards I was given really detailed notes on what to do and what would happen and everything went exactly as planned. It was so reassuring knowing that advice was only a text message away and responses were always immediate. I'm two weeks post procedure now and everything has gone back to normal and the recovery is going just as planned. I'm so glad I did this and chose Harley Street Hair Transplant!

Great clinic with friendly and experienced team. I wanted my surgery done in the UK and found the clinic on Google. Process started with virtual consultations with patient advisor and then with the surgeon. FULL costs were given up front, with good finance options available. On the day I designed a new hairline with the surgeon and ended up getting about 1600 grafts iirc, which was good value for money. Clear pre- and post-op instructions given and recovery has gone as expected. Now waiting patiently for regrowth :) - Grant
